Initial Consultation (Cash Patients)

We ask all new patients to arrive 30 minutes prior to their scheduled appointment to complete any required check-in steps.

A link to complete your forms electronically will be included in your SMS appointment reminder, allowing you to fill them out prior to your visit for a more efficient check-in process.

Please bring the following to your first appointment:

  • Valid photo ID (driver’s license or passport)
  • Method of payment (cash, check, Visa, MasterCard, or American Express)
  • Full list of current medications (bring bottles if possible)
  • Relevant imaging or medical records (X-rays, MRIs, CT scans, reports, etc.)
  • Any questions or concerns you would like to discuss with your provider
