Initial Consultation

We ask all new patients to arrive 30 minutes prior to their scheduled appointment to complete any required check-in steps.

A link to complete your forms electronically will be included in your SMS appointment reminder, allowing you to fill them out before your visit.

Please bring the following to your first appointment:

  • Valid photo ID (driver’s license or passport)
  • Current insurance card and any required authorization or referral
  • Method of payment (cash, check, Visa, MasterCard, American Express) for co-pays or deductible responsibility
  • Full list of current medications (bring bottles if possible)
  • Relevant imaging or records (X-rays, MRIs, CT scans, etc.)
  • Questions or concerns to discuss with your provider

Follow-Up Visits

Please bring the following to each follow-up appointment:
