Do I need a Referral to Schedule an Appointment?

If you have a PPO insurance plan, you do not need a referral to schedule an appointment with our clinic.

If you have an HMO plan that is part of a medical group, a referral from your primary care physician (PCP) is required before your visit.

How Can I Schedule an Appointment?

Scheduling is simple and convenient. You can:

  • Call our office directly
  • Submit a request through our website’s appointment form
  • Ask your referring provider or attorney to send us your information
  • Request a follow-up appointment during any office visit

Our team will help you choose the earliest and most convenient appointment time available.

What Should I Bring to My First Visit?

To ensure a smooth and efficient visit, please bring:

  • A government-issued ID
  • Your insurance card (if applicable)
  • Any MRI, X-ray, or imaging reports
  • Prior medical records related to your pain or injury
  • A list of your current medications
  • Attorney or claim information (for personal injury or workers compensation patients)

Bringing the above items allows our provider to complete a thorough evaluation and begin developing your personalized care plan right away.

Do You Offer Same-Day or Urgent Appointments?

Yes. We understand that pain can become severe or disruptive without warning. Whenever possible, we offer same-day or next-day appointments for urgent cases, including personal injury, acute flare-ups, and new injuries.

Availability varies by location and schedule, so we recommend calling our office directly for the fastest assistance.

What Is Your Cancellation or Rescheduling Policy?

We kindly ask patients to notify us at least 24 hours in advance if they need to cancel or reschedule an appointment. This allows us to offer that appointment time to another patient in need. Late cancellations or missed appointments may be subject to a fee, depending on the type of appointment or procedure scheduled. Our team will review these details with you during
booking.

Do You Offer Telehealth Visits?

Yes. We offer telehealth consultations for appropriate cases, including follow-ups, imaging reviews, medication checks, and certain pre-procedure discussions. Telehealth visits provide convenience for patients who may have mobility challenges, busy schedules, or live farther from the clinic. Some appointments such as physical examinations or procedures must still be completed in person. Our team will help determine whether telehealth is suitable for your visit.

What If I don’t Have Insurance?

If you do not have insurance or prefer not to use it, we offer straightforward cash-pay pricing for your convenience.

  • New Patient Consultation: $500
  • Medication Refill Visit: $250

Payment is due at the time of service. These rates apply to office visits only and do not include procedures, imaging, or additional services.

If you have questions about cash-pay options or need help determining which visit type applies to you, our team is happy to assist.

Are Regenerative Treatments Covered by Insurance?

Most regenerative treatments — including therapies such as PRP (Platelet-Rich Plasma) and other biologic procedures — are not covered by insurance at this time. These services are generally considered elective or investigational by insurance carriers.

Patients interested in regenerative treatments can still receive them on a self-pay (cash-pay) basis. Our team will review costs with you before scheduling any treatment.

Will I Know the Cost of My Treatment Before My Procedure?

Yes. Our team will always review the expected cost of your treatment before it is performed. We want you to feel informed and comfortable with your care.

Once your provider recommends a treatment, our insurance team will:

  • Determine your coverage and out-of-pocket responsibility
  • Obtain prior authorization, if required by your insurance plan
  • Confirm whether the treatment is covered or cash-pay

After this review, we will clearly explain all costs to you — including copays, deductibles, and any non-covered services — so you know exactly what to expect before moving forward.

You will always have the opportunity to ask questions and approve the treatment plan prior to scheduling.

Do You Offer Payment Plans or Financing?

Yes. We understand that medical expenses can be challenging, and we do offer payment plan options for patients experiencing financial hardship.

If you are unable to afford your patient responsibility after your insurance has paid its portion, please let your provider know. They will review your situation and discuss available payment plan options based on your individual needs.

Our goal is to ensure you receive the care you need while supporting you in a way that is manageable and respectful of your circumstances.

How Does the Lien Process Work?

If your care is being handled under an attorney lien or letter of protection (LOP), we will coordinate directly with your attorney regarding payment for services.

Here’s how it typically works:

  1. Your attorney signs our lien agreement, allowing us to provide treatment without upfront costs from you.
  2. You receive care throughout the duration of your case, including evaluations, procedures, follow-ups, and any necessary reports.
  3. Once your case settles, your attorney will resolve your medical bills through the settlement proceeds.
  4. Our office receives payment directly from your attorney after the case is finalized.

This process ensures you can receive the care you need without delays or financial concerns during your case.

Can I Still Be Treated if I Don’t Have an Attorney Yet?

Yes. You do not need an attorney to receive medical care. If you choose to hire an attorney later, we can update your case file and coordinate with their office immediately. If you are unsure where to begin, our team can help explain the process and what information attorneys typically need without providing legal advice.

Will My Treatment Affect My Settlement?

Referrals can be submitted quickly and securely through any of the following methods:

  • Email: Providers or attorneys can email referrals and records directly to our office.
  • Fax: All medical records, imaging, and referral notes may be faxed to our dedicated intake line.
  • Phone: Providers may also call our office to coordinate urgent or same-day referrals.

Our intake team reviews all referrals promptly and contacts the patient directly to schedule.
